Private Medical Examinations
Medical examinations for special purposes such as pre-employment, fitness to travel, elderly drivers, fitness to undertake sports etc, may require longer than a routine appointment time.
Please check the fee chargeable when you require to arrange any such examination.
Registering with the practice
When you attend the Practice to register you will be asked to complete a GP registration form, and new patient questionnaire form, these must be completed before you see the Doctor, so it is advisable to attend at least 10 minutes before your appointment time, so that you can compete the information required.
Request for GP Letters
If your Doctor has agreed to write a letter for you in connection with your employment, acting as a referee, or for some other specific area, you may be charged for this letter.
Please allow at least two working days before collecting your letter

What should I do if I am late for my appointment?
Changes

are held on a computer, there may be an administration charge.
For a copy of older paper records, and results such as X-rays, you may have to pay photocopying and
administration charges.
Can I cancel my appointment?

attend, you will be sent a letter informing you that you have defaulted from your appointment. Persistent
defaulters maybe removed from the list.
You can also complete our appointment cancellation notification request form. This can only be used if your
appointment has been arranged for more than 24 hours in advance. (excluding weekends and public holidays).
